Acknowledgement

I acknowledge that end-user/representative must be available at conclusion of startup for user training. Any return trips are billable.

I acknowledge the above items will be completed as indicated. If the UPS system is not ready for startup or FAA personnel are not available for training at the time the Service Technician arrives on-site, a task order written by an AUTHORIZED AGENT OF THE FAA may be required to reschedule a Service Technician for the startup. A new pre-startup checklist must be completed for the new service request.

We request 10 business days from the day that we receive the completed checklist to schedule start up services. Prior to the startup date, a technician will coordinate with the site point of contact to confirm the date and time.
